3) Monk’s identification card (for Thai)

4) Medical Certificate

5) Official Photo (size 1.5 inches) 3 copies

6) Certificate of Dhamma Studies (if available)

7) Certificate of first name or surname change (if available)

8) Official English proficiency test result certificate (if available)
